THAME COMMUNITY HOSPITAL
Founded in 1897 as the Victoria Nursing Home, Thame Community Hospital serves Thame and its surrounding area in Oxfordshire and Buckinghamshire, for example providing care for patients following treatment at an acute hospital. Managed by Buckinghamshire Healthcare NHS Trust, the hospital acts as a bridge between hospital and home, particularly for patients needing rehabilitation before returning home.
IN-PATIENT CARE: The Inpatient Unit provides eight beds for patients admitted from home, for rehabilitation following care in an acute hospital, for palliative and for terminal care.
THAME DAY HOSPITAL runs two days a week and provides a multi-disciplinary rehabilitation programme,for patients at risk of falling, for those with complex needs and following discharge from hospital.
OUT-PATIENTS: A range of specialist clinics are provided by visiting professionals from local hospitals, enabling patients to access these services locally. A Musculoskeletal Physiotherapy Service is based on site, as are Health Visitors and Podiatrists. Wykeham Park Day Hospital, managed by Oxfordshire and Buckinghamshire Mental NHS Foundation Trust, provides a service for people with mental health issues.
ADULT COMMUNITY HEALTH CARE TEAM:
ACHT is a multidisciplinary team of District Nurses, Physiotherapists and Occupational Therapists, supported by Assistants. The team operates 24 hours, seven days a week, and reflects an integrated approach designed in response to the needs of the local population, aiming to prevent the need for admission to hospital, maximise independence, and facilitate discharge from hospital.
THAME AND DISTRICT DAY CENTRE: meets at the hospital three days a week for fifty weeks of the year. It offers social contact, a hot meal and a range of activities for those over-65s who might otherwise be socially isolated or housebound. The Centre is part funded by Oxfordshire Social Services but relies heavily on fundraising support from the local community.
